**Remember a specific Sports physical is needed for most HS sports/activities. These physicals are good for 3 years, but must be valid through the entire season (through the state meet) in order to be cleared. Most kids get a sports physical before 7th and 10th grades. Please see the tab "Registration & Forms" above if you need to print a form for your doctor, otherwise they may have their own clearance form for you and that works for us as well. During the summer is the best time to get your physical so it does not interfere with any particular season. Smart School K-12 will NOT allow you to sign up if you are in need of a current physical. Please turn in your sports physical clearance sheet to the school your child will be attending before 3:00pm. They will enter your physical and clear you to sign up for your sport/activity.
Or please email the sports physical clearance sheet to the following:
7/8th graders: email@example.com
9-12th graders: firstname.lastname@example.org
